Fact: The drop rate for this is roughly 1%.
Falsehood: It doesn't have to be Heroic. It also drops in Regular.

Regarding bursting Millhouse 3 times in a row like was done in the first group - This isn't possible since Patch 4.0.6, because if you make millhouse retreat from the third group with the secound one still living he won't run down to trigger the boss but return to the second group. Thats why you have to kill the 3rd entirely before burning him down to 50% hp in the second group.
You can't just use camo all the way to slabhide because Milhouse Manastorm has to trigger the first boss.
